To enroll in a medication administration course please email all documents to histtraining@gmail.com.
In the email please include:
- Agency name
- Medication administration Application(s) filled out completely including all signatures and check boxes
- Attach each staff skills check off sheets or the list of skills with their application
- Ensure the correct box is marked for the course they are needing to renew or certify in
- Ensure the application is signed in all areas
To enroll in a CPR/First aid course please email the following information to histtraining@gmail.com:
- Your name or the name of your agency
- How many people will attend
- Type of Course (example. cpr or first aid, or both)
- Include the correct spelling of each attendee’s name
